There is some comfort, it seems to me, that even the first century church had problems with, for lack of a better term right now, hypocrisy.
Given the context, I believe the Bible’s first target for this passage would be the preachers and teachers of God’s Word – the masters of verses 1-2. We do indeed offend all in many things. It is a necessary consequence of preaching the Word of God that there will be people who become offended at the truths we preach. As the old preacher used to say, “When I first started preaching, I was a afraid I would offend people. Now I am afraid I won’t.”

On the other hand, it ought not so to be that the same mouth that preaches the truth from God’s word speaks things that are error, or hurtful and mean spirited or that would lead soul to turn away from God.
Apparently, this was a problem in the first century churches. Not much has changed.
